The management of premaxilla in cleft lip and palate patients is a challenging process. In bilateral cleft the premaxilla is carried forward where it has only one restraining connection to the vomer. Dealing with such difficult situation led to the appearance of variety of techniques and options for treatment. These options include amputation of the premaxilla, presurgical orthopedics, premaxillary setback and distraction osteogenesis. Although of all available studies, the management of the premaxilla in cleft lip and palate patients is still controversial. Modification of the management protocol sometimes is necessary depending on the severity of the case as well as the time of presentation.
