The College of Applied Medical Sciences Ethics Committee

In order to process your IRB application quickly and efficiently please provide the following items as a single PDF file. The applications have to be submitted at least three weeks from the end of the first and the second semesters.

1) A signed letter from the principle investigator requesting the ethical approval for his/her research project.
2) Complete the IRB application form:
a- Researcher’s names and the title of the project should be inserted in both Arabic and English in Page 1.
b- Complete Arabic summary section (Section 3).
c- No budget, otherwise the application has to be sent to KSU Main Ethics Committee at the College of Medicine.
d- The form should be signed by all researchers in Page 1 and by the principle investigator inside the form.
e- Clearly indicate how long is needed for the project to be completed, six month, for example.
f- Complete the Table with ticks corresponding to each task.
3) One page as a study protocol that shows what the researchers will do from the start till finish.
4) A copy of the consent form, if human subjects involved.
5) A copy of any questionnaires, if applicable.
6) CVs for all researchers.
7) A Word file that contains the following (in both English and Arabic):
a- The title of the project.
b- The names of all researchers.
c- The period needed to conduct the research in months.
