Eye Trauma

young male 25 years old , hit into his face with chair. lost his right eye vision.

diagnosis : Ocular globe rupture

clinical symptoms:
 
60% will have acuity better than 6/12

shallow anterior chamber

irregular pupil

colored spot of choroid visible on sclera

small subconjunctival haemorrhage

decreased intraocular pressure

cloudy lens due to exposure to air

 

Investigations:

 

CT : look for foreign body (FB)
Ultrasound of the eye
Plain radiography if above not available

Management :

Do not attempt to remove FB
Eye shield (not pad) 

Anti-emetics (Ondanstron 4mg IV ) 

No topical antibiotics (especially ointment)

IV cephalothin and gentamicin

-15% incidence of endophthalmitis

tetanus prophylaxis
